Appointments and Office Policies

To ensure your dental appointment at “Midtown Family Dental” is pleasant, please take a moment to familiarize yourself with our office policies and procedures.
Due to the Canadian Personal Privacy Act (see our privacy policy), we are unable to access any specific information from your dental insurance company regarding your dental plan. It is your responsibility to know the parameters of your coverage, such as annual maximums, frequencies, renewal dates, and any other limitations.

We also extend the option of direct billing (also called assignment) for special considerations. However, your insurance company must accept this service.
Please understand that it is your responsibility to keep track of your upcoming dental appointment. As a courtesy, we send out reminder emails to you before a requested appointment and make confirmation calls or send text reminders There are many times when our patients require urgent or emergency treatment and therefore require an appointment as soon as possible.

Ideally when patients give our clinic advanced notice to rebook a scheduled appointment, this time, can, in turn, be allocated to those patients in need of urgent treatment. In this way, the clinic can best serve the needs of ALL patients.
Bearing these special needs in mind, our clinic requires a minimum
of 2 business days’ ( 48-hrs prior) notice if an appointment needs to be rescheduled.
At “Midtown Family Dental”, our goal for you is to have a pleasant experience in our office. We strive to help you to the best of our ability in helping you attain a high level of dental and overall well-being.
If you have any questions, please do not hesitate to contact us at info@midtownfamilydental.ca
